Preparing For Your Appointment
Once your appointment is scheduled, you’ll have the option to receive your New Patient Registration Forms by email or mail in advance. To save time during your visit, you can also fill out these forms online before arriving.
What to Bring
At your appointment, please bring the following essentials:
- Your government-issued photo ID
- Your insurance card (if applicable)
Our front desk staff will scan these documents into your patient chart. Additionally, let us know if there are any updates to your personal information, such as your address, phone number, or email, or if you’re switching insurance providers.
During Your Appointment
When you arrive, our friendly staff will assist you in checking in. If you haven’t completed the new patient registration forms online, you’ll need to fill them out upon arrival.
We will also:
- Verify your insurance (if applicable) prior to the appointment.
- Collect any necessary payments during your visit.
- Take an initial photo for your patient records—this is securely stored in compliance with HIPAA guidelines.
Once you’re finished with necessary paper work, our dental assistant or hygienist will take you back to take your x-rays.
After your appointment, our front desk team will assist you in booking future visits and collect any additional payments if necessary.
