With our in-house savings plan, patients receive 20% off all services as well as:
- 2 Cleanings
- 2 Periodic Exams
- 1 Pano or Full-Mouth x-ray
- Bitewings every 6 months
- Fluoride Treatments every 6 months
- Unlimited Emergency Exams and X-Rays
Periodontal patients have the option to add $150 to their plan. Services include:
- Scaling and Root Planning $150 per quadrant
- Perio-Maintenance $99
Add a family member to your plan for $269.10
Collingswood Family Smiles’ plan is simple and hassle free. Just pay an annual enrollment fee and you are on your way to great savings!
Exclusions and Limitations
Exclusions and Limitations
- This is a savings plan & NOT a dental insurance.
- Periodontal add-on is an additional cost PER patient.
- This cannot be combined with any other dental insurance.
- This is ONLY good and cannot be used at any other office but Pitman Dental.
- If you are referred out, you cannot use this savings plan at another office.
- Payment in full is due at time of service.
- This in-house savings plan is NON-REFUNDABLE: No refund will be given if patient chooses not to use the savings plan.
- This in-house savings plan is subject to change yearly.
- Should there be dental treatment needed following any type of accident/injury where a lawsuit is required (workman’s comp), this savings plan cannot be used.
